- Tear-off layers removed, and disposal by load or tonnage
- Decking replaced by sheet count — the single most common supplement
- Underlayment, ice and water shield, drip edge, flashing and vents, itemised
- ACV paid, recoverable depreciation outstanding, and the homeowner deductible, shown separately
- Supplements approved since the original scope, each with its approval date
- Permit, inspection and the workmanship warranty term
