This invoice should carry
- Invoice number, issue date, due date and the pour date
- Cubic yards ordered and cubic yards placed, shown separately
- Mix design and PSI, with any admixture — fibre, accelerator, air entrainment
- Short-load or minimum-load fees charged by the plant, passed through
- Formwork by linear foot, and reinforcement by mesh area or rebar weight
- Finish type — broom, trowel, exposed aggregate, stamped — priced per square foot
- Pump or conveyor hire, if the truck could not reach the pour
- Crew hours on pour day, and any standby time waiting on trucks
