- Invoice number, draw number, period covered and the contract reference
- Stage being billed: demolition, rough-in, close-up or finish
- Percent complete by trade, and the value claimed this draw
- Allowance reconciliation: allowed, actual, and the difference per item
- Fixtures installed by model, and who supplied each
- Concealed conditions found, billed at the agreed unit rates
- Approved change orders with their numbers and dates
- Retainage withheld, if any, and the condition for its release
