This invoice should carry
- Invoice number, issue date, due date and the property address
- Each tree listed separately: species, approximate height, and what it overhangs
- Removal, reduction or deadwooding, stated per tree
- Equipment used — crane, bucket, chipper — with hire time
- Crew size and hours, with climber time separate from ground crew
- Stump grinding by count and diameter, and the grind depth
- Debris: chipped on site, hauled away, or logs left for the customer
- Certificate of insurance reference, and any permit for protected trees
