- Invoice number, issue date, due date and the areas treated
- Square footage per area: attic flat, slopes, knee walls, rim joist, crawlspace
- R-value installed per area, stated explicitly
- Material and method, with bag count or installed depth as evidence
- Air sealing performed, itemised rather than described as included
- Baffles fitted, and attic ventilation intake and exhaust
- Existing material removed and bagged, where applicable
- Rebate or utility programme reference, where the customer is claiming
